A serious disaster in the history of the Wuppertal monorail occurred due to the fault of workers who forgot to remove the shoe from the rail after night preventive maintenance. As a result, the car went off the rail and crashed from a height into the Wupper River. Five passengers died, 49 were injured of varying severity.
